Executive Overview We invited one of the two authors of “Hubble Error, Time, Money and Millionths of an Inch.” published in our November 1993 issue, to do a follow-up on the Hubble fiasco. We wanted to know if NASA had learned anything from its mistakes. We also wanted to know if the lessons learned had been taken seriously or quickly forgotten. Editor
